Have you ever wondered how healthy you are? What your health risks are? What you
can do to become healthier?
If you answered “yes” to any of the above questions, HMSA can help you.
Your first step is to complete the online
health risk assessment questionnaire
that will evaluate your health and identify your health risks. Your information
will be analyzed, and a wellness action plan that is specific to your needs and
goals will be generated.
If you prefer to complete the questionnaire on paper, simply call our HealthPass
staff.
By completing this online questionnaire, you may be eligible for HMSA benefits that
include the HealthPass screening and health education classes at no additional cost.
Take advantage of the following services offered through HealthPass:
- A HealthPass biometric screening and consultation.
- Telephone consultations and health coaching.
- Preventive examinations.
If you have completed your health risk assessment questionnaire and are eligible
for HealthPass, schedule an appointment for your biometric screening. We will measure
your blood pressure, height, weight, body mass index, blood cholesterol, and blood
sugar, and do a body fat analysis. A health professional will discuss your results,
map your wellness goals and personal plan with you, answer your questions, and discuss
lifestyle changes you would like to make.
Your health risk assessment provides an impetus for positive change. Use the provided
tools and strategies to reach your personal health goals and succeed!
